The National Committee for Quality Assurance (NCQA) offers programs to providers that are interested in ensuring quality within their offices. One of the many programs that NCQA offers is the Patient-Centered Medical Home (PCMH) recognition program, which is a model of care that emphasizes care coordination and communication to transform primary care into “what patients want it to be”.

NCQA PCMH Recognition is the most widely adopted model for transforming primary care practices into medical homes. Research confirms medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.

Once a provider becomes NCQA PCMH recognized they will be eligible for certain per member/per month(pm/pm) payouts from the health plans that recognize NCQA PCMH status. Also, recognition persists for three years.

UOP continues to encourage our providers to incorporate the triple aim philosophy into their practices. With the NCQA PCMH program, providers will find it beneficial to utilize their knowledge of the triple aim philosophy, which is to decrease healthcare costs, increase quality, and better patient satisfaction.
